Leave of Absence
At Rehab Industries, we understand that life happens and you may need to take time off to address personal circumstances such as health issues, military service, or family-related events like childbirth or adoption. When these things occur, you may be eligible to take a Leave of Absence (LOA).
What You Need to Know
When taking LOA, PTO will run concurrently with leave time. Leave can be continuous or intermittent. If you are requesting continuous leave then it must be for a single, uninterrupted period that has a pre-determined end date. Intermittent leave is a reduced schedule leave taken in separate blocks of time. Each leave case is considered on a case-by-case basis.
Examples of Continuous Leave:

Recovering from surgery

Maternity/paternity leave
Examples of Intermittent Leave:

Flare-ups from a chronic condition

Frequent doctor visits to address ongoing health concerns
Please note: Both types of requests require medical certification from a healthcare provider.
Who’s Eligible for FMLA?
Rehab Industries employees who meet these requirements:

Must be employed at a location with 50 employees within a 75-mile radius

Must have been with a Rehab Industries company for at least 12 months
-
Must have accrued at least 1,250 hours of service within that 12-month period
